The Position:

The Community Connector plays a key role in enhancing the quality of life for developmentally challenged adults in a residential setting by assisting with educational, social and recreation needs, in addition to organizing and carrying out recreation activities. Under limited supervision, the Recreation Programmer will assist residents to and from planned programs and organize programs such as outings, entertainment, crafts, parties etc. Reporting to the Recreation and Development Manager (the Manager), the Recreation Programmer works within the division to support operations by demonstrating a hands-on and supportive Persons-Centered approach to resident care. 

KEY ACCOUNTABILITIES

 

1.   General Duties

  • Maintain effective communication with dignity and respect with residents and fellow team members
  • Participate in monitoring quality
  • Responsible for daily recording and charting as per Group Home policy
  • Maintain resident confidentiality
  • Support quality of life for residents

 

2. Resident Programming

  • Utilize daily living activities for residents as elements of programming goals
  • Work with the Manager to complete recreation assessments for each resident
  • Facilitate resident involvement in all activities
  • Be creative in resident participation in the community
  • Pass on pertinent information to supervisor
  • Implement individual care plans
  • Complete monthly budget for each resident per month
  • Create and maintain the recreation calendar
  • Provide support to individuals during recreation or community-based activities
  • Take initiative to ensure residents have adequate recreation and community-based outings outside of the regular recreation schedule
  • Plan, organize and deliver regular recreational events, activities and programs (ie: birthday parties, baking, holiday events)
  • Plan and organize special recreational events (ie: yearly camping trip, holiday events, fundraisers)
  • Liaise with outside agencies to provide additional programs suitable for residents
  • Ensure recreation activities/events meet the ability of the residents and match their cognitive/physical needs
  • Encourage and assist with resident interaction during activities
  • Ensure supplies are available for activities
  • Decorate activity area to suit occasion and clean up afterward
  • Record attendance for all recreational programs
  • Maintain communication through calendars and month-end reporting
  • Contacts and involves resident’s families in activities and events
  • Create and promote activities through posters and information bulletins
  • Promote good working relations with all residents and maintain confidentiality
  • Pre-plan for recreation outings/events for days when not on duty

 

QUALIFICATIONS

  • Grade 12
  • Post-secondary education in a related discipline (ie: Therapeutic Recreation Diploma, Kinesiology Degree, Disability Support Professional etc.) or a related field
  • Successful completion of Transferring Lifting Repositioning (TLR) is required
  • Successful completion of Professional Assault Response Training (PART) is required
  • Valid driver’s license and the ability to drive a wheelchair bus

 

EXPERIENCE

  • Experience working with developmentally challenged adults in a residential setting
  • Experience working with residents with high medical needs
  • Experience planning and coordinating events is considered an asset
  • Experience in budget development and management is considered an asset

 

KNOWLEDGE, SKILLS & ABILITIES 

  • Is committed to a Person-Centered approach to providing care
  • Ability to fluently read, write, speak and understand English fluently
  • Ability to manage a budget and assist in the development
  • Ability to work with developmentally challenged adults
  • Ability to plan, implement and coordinate activities, outings and functions for residents
  • Able to deal with unpredictable behaviours
  • Strong organizational and time management skills
  • Ability to operate and train in the use of the wheelchair van, motorized wheelchairs and audiovisual equipment
  • Proficient with a computer and required software
  • Demonstrates empathy, concern and interest in residents
  • Ability to adapt to varying settings and situations
  • Ability to work with minimal supervision
  • Ability and willingness to work days, evenings, weekends and statutory holidays
