The Position:
The Social Worker is an integral member of the care team at Luther Special Care Home, providing psychosocial support, case management, assessment, and advocacy services for residents and their families. The role plays a vital part in enhancing the emotional and mental well-being of residents, ensuring dignity, independence, and quality of life throughout their care journey. As a champion of resident-centered care, the Social Worker supports transitions into care, facilitates care planning, and promotes a respectful, inclusive, and therapeutic environment that aligns with the mission and values of LutherCare Communities (LCC). This position works collaboratively with interdisciplinary teams to address mental health concerns, navigate complex family dynamics, and support program development that responds to resident needs.

- Advocate for a resident-first approach in all interactions and care decisions
- Maintain compliance with professional, ethical, and regulatory standards
- Promote interdisciplinary collaboration and foster inclusive communication
- Prioritize emotional safety and dignity for residents and families
- Demonstrate empathy, cultural competence, and trauma-informed care
- Actively contribute to a safe and respectful work environment
- Support the strategic goals of LCC through innovation and leadership in resident advocacy
WHAT YOU WILL BRING TO THE POSITION & ORGANIZATION
Education:
- Bachelor’s degree in social work
Licensure:
- Active and regular membership with the Saskatchewan Association of Social Workers (SASW)
Experience:
- 2-4 years of experience in gerontology, long-term care, or healthcare-related social work, case management
