 THE POSITION

The values of the organization require that all positions support the mission and values of LutherCare Communities. The Nurse assumes responsibility for enhancing the care of the residents in the Group Homes.  including providing nursing functions appropriate to the resident to meet the physical, and psychological needs of residents. Responsibilities include providing nursing functions within the Nursing Scope of practice appropriate to resident needs, maintaining individual care plans, supervising the disability support workers, and collaborating with the team lead, Group Home Coordinator as well as families. 

ACCOUNTABILITIES

  • Supervise, evaluate, and monitor resident care within the Group Home
  • Comply with all licensing requirements set by Community Living service Division.
  • Ensure all OH&S regulations are abided by.
  • Advocate on behalf of residents if and when necessary.
  • Take part in resident functions and activities to maintain visibility and cultivate a close relationship with residents and their families.
  • Identify and communicate resident needs to the interdisciplinary team.
  • Contact external agencies for referrals as needed, i.e.: dietician, OT/PT, CLSD social work.
  • Assess medication safety in the home, and review medications from pharmacy.
  • Follow up with physicians as needed.
  • Assess resident medical protocols, and ensure they are updated as needed.
  • Abide by Policies and Procedure of LutherCare Communities

QUALIFICATIONS

  • Bachelor of Science in Nursing (BScN) and current registration with the College of Registered Nurses of Saskatchewan. (CCRNS)
  • Two to Five years’ experience in working in a community setting or acute or long-term care.

KNOWLEDGE, SKILLS & ABILITIES

  • Ability to fluently read, write, speak, and understand English.
  • Ability to work effectively with cognitive and physical disabilities, with high medical needs.
  • Ability to demonstrate empathy, concern, and interest in providing quality care to participants.
  • Ability to implement the individual Program Plan.
  • Good interpersonal skills in communication and human relations
  • Excellent administrative, organizational, and interpersonal skills
  • Knowledge of word processing and database programs
  • Ability to show independent initiative.
